Northwest Hospital & Medical Center offers a wide variety of outpatient rehabilitation therapy services at two locations – the hospital’s main campus located at 1550 N. 115th Street, and the Northwest Outpatient Medical Center located at 10330 Meridian Ave. North. Outpatient services are performed by licensed physical therapists (PT), occupational therapists (OT), and speech language pathologists (SLP).

Outpatient Programs
Outpatient services require a referral from a physician or healthcare provider.
